Child Disability Team
The Neath Port Talbot Social Services Child Care (Disability)
Team work with disabled children and their families with a view to
providing services to assist in minimising the effects of a child’s
disability, and to give disabled children/young people and their
families the opportunity to lead lives that are as normal as
possible.
The Child Care (Disability) Team receive direct referrals, and
following an assessment of need can provide a number of services
that can be available to disabled children and their
parents/carers.
These services include sitting services, personal care services,
Outreach services and short-breaks with link carers or in
specialist residential units. Shared care with specialist foster
carers can also be arranged.
Other services we can help families access include help with
challenging behaviour, play and leisure, Welfare Rights advice,
Portage, therapeutic services (Physiotherapy, Occupational Therapy
etc), specialist placements and foster care.
Our Specialist Health Visitor and Community Nurse can also
assist with co-ordinating children’s health needs and offering a
comprehensive continence service.
We work with children and young people from birth to eighteen
years if they are physically or sensory impaired, or have physical
and/or learning disabilities and:-
- If their disability means they have significantly greater
difficulty with physical, intellectual, social, emotional or
educational development than the majority of children of the same
age.
and
- If these difficulties are such that the child requires Social
Work assessment/care planning by the Child Care (Disability)
Team.
