Services
Neath Port Talbot provides a range of services to its public in
conjunction with the Local Trust, the Local Health Board, voluntary
and private sector.
As already mentioned, the Community Support Team employs a range
of specialist health workers along with clinical nurses trained to
deal with particular problems related to learning disability
clients.
There is also a Person Centred Planning coordinator who is
responsible for introducing a new way for involving clients in what
services they receive.
There is a Transition Officer, who works closely with our
clients in the Child Disability Team. She begins her involvement
with young people as young as 14 years of age and is responsible
for working closely with our colleagues to prepare young people for
their move into adult services at 18 years of age.
Finally we have a Housing Options Coordinator, the only such
post in Wales. Her role is to work closely with clients and their
families to look at a wide range of accommodation options for their
future support.
Direct Services
Neath port Talbot has nine day services which provide a variety
of day time opportunities that cater for a wide range of adult
requirements; they include:
Three Special Needs Day Services that provide high levels of
support for people with profound learning disability, many of whom
may also possess physical and/or sensory disabilities. These are
located in Port Talbot, Cadoxton and Neath Abbey.
There are four local Day Services that assist people in
accessing the opportunities that exist within their local
community. These are situated in Port Talbot, Glyncorrwg, Neath and
Pontardawe.
There is also an alternative Day Centre in Neath that provides a
more relaxing atmosphere for older people with a learning
disability.
At Neath Abbey Industrial Estate, we have a Vocational Skills
Centre that provides structural work, training opportunities and
qualifications for those who may wish to move on into integrated
employment. They are involved with a number of community projects
and work in partnership with other agencies and companies.
These services provide support to approximately 280 people
within the borough.
We also have a Home Support/Tenant Support Team that provides
support for people with a learning disability who live
independently in the community or at home with their carers.
We have a residential respite unit at Cimla in Neath and a
larger unit at Port Talbot that provides respite care and short to
medium term residential care.
The Authority also works closely with a number of voluntary
organisations who either work directly, or assist in representing
the views and promoting the rights of people with a learning
disability.
There are also Parent/Carer Groups on Advocacy Service and
parent, carer representation within the Authority structure.
The take up of Direct Payments has been significant in the last
few years as has the application of the Independent Living
Fund.
Finance
Some of the services provided by Neath Port Talbot to the client
incur a financial charge and are for a fixed amount. Others are
discretionary and on a scale depending on how much savings you have
and what your income is. These will be explained to you at an early
stage by your care manager.
Residential
Care
A number of clients for various reasons are unable to live at
home on their own and need high levels of support. In conjunction
with the private sector, we fund packages of care to enable people
to live in residential care settings usually as close to their
homes as possible in order that they can maintain contact with
family and friends.
Communication
If your spoken English is not very good, or you prefer to
discuss matters in Welsh, we can arrange for interpreter facilities
by telephone. To use this service we would need your name and
telephone number (in English) and the language that you speak. We
will then call you back as soon as we can with an interpreter with
whom you can converse.
Confidentiality
We will keep written and electronic records about the
information you provide. We will keep this information secure and
only share it with people who will provide your care, or to protect
other people. We will normally ask your permission before sharing
this information.
Comments and
Complaints
We welcome feedback from you on the services we provide, whether
negative or positive. It helps us to know what we are doing well
and what we are doing not so well. If you have a complaint about
our services, we have officers you can speak to whose job it is to
manage complaints.
